ABOUT HORIZON SOUTH

Horizon South of San Jose, Calif. is a gender-specific, residential treatment program for men struggling with chemical dependency and co-occurring disorders. It is one of six facilities operated by Horizon Services, Inc., a treatment provider serving residents of Alameda, San Mateo, and Santa Clara Counties. Horizon Services, Inc. also operates Project Eden, a program designed for adolescents, and a women’s-only residential facility called Chrysalis. The facility offers non-medical detoxification services on-site.

TREATMENT & ASSESSMENT

Clients at Horizon South participate in a face-to-face assessment and screening before beginning treatment, in order to ensure that the residential program is appropriate for their needs. This assessment helps clinicians create an individualized treatment plan. If necessary, clients may undergo a non-medical, social detoxification phase of treatment. During this phase, clients are closely monitored to ensure they detoxify from substances comfortably and safely.

Once stable, residents engage in Horizon South’s Adult System of Care (ASOF), which utilizes evidence-based treatment methods. The program includes a highly-structured schedule of group and individual counseling sessions, as well as educational presentations. The facility also incorporates the 12-step model of recovery into programming.

Individual counseling sessions may address issues common among men struggling with chemical dependency, such as anger management, stress management, and improving self-esteem. The facility may also provide specialized group sessions for individuals with co-occurring mental health concerns. Referrals to community-based mental health clinics for psychiatric evaluations and medication management may also be provided.

Through educational sessions, clients learn about the disease of addiction and relapse prevention techniques, such as trigger avoidance, developing support networks, coping skills, and improving life skills. Family participation is emphasized, and loved ones may attend counseling sessions, educational seminars, and support groups.

Upon discharge, clinicians assist clients with referrals and discharge planning to support their transition back into the community. This may include referrals to sober housing facilities, less-intensive outpatient programs, vocational training and educational opportunities, and other community-based resources.
